Registration Forms
You will need to complete the following forms in order to register your child as an RMSC player. The forms must be completed for each child registering.

- Player Registration Form
This form MUST be signed by the Parent/Legal Guardian OR a Player age 18 or over. Players who turn 18 during the seasonal year must have a new form filled out. Players who are 18 MUST sign their own form. Two copies of this form must be turned into your League/Club Registrar with the appropriate fees. You may make a copy of the signed form and turned the original and the copy in or you may turn in two signed copies of this form.

Parent / Caretaker Agreement Form to be completed by all parent or guardians. One for each registered player is required.
